Sepsis form

The Sepsis forms are an important tool in the fight against Sepsis. All patients who NEWS score 3 or more with signs of new infection or have suspected sepsis need to have a form completed. These forms can be located in your sepsis box or trolley. 

 

This data can be viewed using the dashboard link below. Entering the information via the e-form allows you to record compliance with the Sepsis 6 bundle; allowing wards and hospital to then focus on areas that need to be improved to support better outcomes. 

Please note that to access e-form and dashboard you need to be using a health board device
